After School Program Waiver and Release

PARENT'S AUTHORIZATION & WAIVER FORM

PHOTOGRAPHS AND SOCIAL MEDIA:

I give permission to Gunpla After School (Negative Split Technologies, LLC) and Makers United Clubhouse (Clubhouse)  to use photos taken of my child at Clubhouse as well  as photos of my child's craft projects to be used for marketing on Gunpla After School’s website, and  social media, including Facebook, Instagram, TikTok and X.  

ABILITY TO ENGAGE IN CREATIVE ACTIVITIES AND ASSUMPTION OF THE RISK:

Gunpla After School and Clubhouse take all possible precautions to reduce risk and provide safe, healthy and enjoyable experiences. I warrant that my child is able to follow directions for all activities and play in the class, camp and within the space in general from all instructors and team members. I acknowledge that risks from participation in class activities exist and that I have allowed my child to attend knowing these risks and the possible consequences including personal injury. 

WAIVER AND RELEASE OF LIABILITY: 

As a parent or guardian of my child, I agree that I will not hold Gunpla After School or Clubhouse, liable for any personal injury, property damage or loss of insurance. I agree to release and hold harmless Gunpla After School or Clubhouse from all liability incurred as a result of my child's participation in creative crafting classes and/or crafting camp, and/or creative play and that these terms serve as a release for me, employees, volunteers, interns, and property owners. Gunpla After School or Clubhouse is not responsible for any lost property of my child. 

I am the parent/guardian of the child that I am registering for participation in Gunpla After School at Clubhouse.
